Patient Educator (Early Childhood Screening and Development) - Job Overview
Schedule: Monday-Friday (8AM-4:30PM) or (8:30AM-5PM)
Are you passionate about shaping the health and development of young children? At Legacy Community Health, we’re looking for a dedicated Patient Educator with a special focus on early childhood screening and development. In this role, you’ll be a vital part of our pediatric care team, working directly with families to support children’s growth, identify developmental milestones, and promote lifelong wellness from the very beginning.

Key Responsibilities
- Deliver health education programs in individual, co-visit, and group settings with a focus on early childhood development.
- Conduct screenings for developmental delays, social-emotional health, and school readiness using validated tools.
- Support caregivers in setting developmentally appropriate goals and navigating early intervention services.
- Document all interactions in electronic health records and contribute to data tracking for early childhood outcomes.
- Develop and maintain educational materials for families on topics such as speech/language development, motor skills, and positive parenting.
- Participate in interdisciplinary team meetings to coordinate care and share insights on child development trends.
- Contribute to grant-funded initiatives and performance metrics related to early childhood health.
Minimum Qualifications
- High School Diploma or equivalent required.
- 3–5 years of relevant experience; experience in early childhood education, pediatrics, or child development preferred.
- Bachelor’s Degree in a related field with at least 1 year of relevant experience preferred.
- Experience working with families from diverse cultural backgrounds in one-on-one and group settings.
- Familiarity with developmental screening tools (e.g., ASQ, M-CHAT) is a plus.
- Bilingual in English and Spanish strongly preferred.
- Ability to complete point-of-care testing and phlebotomy training within 3 months of hire, if required.
